Office Policy

On the first visit we require a biological parent, or a court-appointed guardian to be present. Before any treatment can begin, we will require the accompanying adult to provide custody paperwork specifically granting them the right to make medical decisions on behalf of the child.

For subsequent visits, the parent/guardian can fill out a form giving permission to other trusted adults, who could bring the patient for all future dental appointments. Without this written consent form clearly stating the name of the accompanying adult, as per the law we cannot treat any patients under the age of 18.

If the patient has not been seen at the office for longer than a year, a new updated health history will need to be completed for which we will once again need the biological parent or court appointed guardian.

This office prides itself on creating a warm, welcoming, and safe environment for children of all ages and offensive language/disrespectful behavior will result in immediate termination of the patient and the family.

Please note we have video surveillance monitoring throughout the office for your safety.

CANCELLATION/ DISMISSAL POLICY

When we book your appointment, we set aside a dedicated time slot just for you and our appointment times are limited to 30-45 minutes. Any patient who is more than 10 minutes late will be considered a “no show” and will need to reschedule.
The office has a strict policy about missed appointments, and you may be charged a missed appointment fee ($25) if you do not call at least one day before the appointment to cancel or reschedule it, and after three missed appointments without a call to cancel or reschedule, the office retains the right to dismiss the patient permanently

FINANCIAL POLICY

If you have dental insurance, we will file a claim on your behalf with your insurance carrier. Please update our front desk with all dental insurances (primary and secondary) that the patient is eligible for on or before the day of appointment. This will allow our staff to check your insurance eligibility/coverage and to the best of our ability assess your out-of-pocket costs. Additionally, we recommend you contact your insurance carrier directly to clarify the covered services, since payments for procedures are due before the appointment can begin.

The parent/guardian present is responsible for paying the deductibles, estimated co-pays and any non-covered services in full at the time of the visit. Any amounts that are denied or unpaid will be billed to you. This is likely in the case of a patient being treated by another dentist in the last 6 months, and we request you to inform the front desk of this possibility. We also encourage you to check with your insurance company regarding specific coverage & limitations.

We offer the option of storing your credit card information securely on file for future payments. Please speak to the front desk for further details.

Accounts 60 days overdue are subject to late fees and collections; agency fees and other charges may apply.

In cases of divorce or custody situations, we will not become involved with mediating financial arrangements between parents, and the parent present at the appointment is responsible for payment due that day.

We recommend that you monitor your children throughout the visit to avoid any accidents or injuries.
